Dr, Javaheri: Dr. Ashkan Javaheri is the head of the Geriatric Division at Mercy Medical Group in Sacramento, CA and assistant professor at UC Davis School of Medicine. He completed his fellowship training at Stanford University in 2008 and since then, he and his team have been providing medical care at several skilled nursing facilities, consult clinics, and Geriatric House Call Program. Dr. Javaheri’s research interest is to promote geriatric care with the help of the new technologies including telemedicine and innovative devices for home monitoring.
James Roxburgh, RN: Jim Roxburgh is the Director for the Dignity Health Telemedicine Network (DHTN). He is responsible for the leadership, development and coordination of the DHTN. The DHTN currently has 30+ Partner Sites (Hospitals) providing telehealth services in the acute, ambulatory and home setting. Jim’s undergraduate degree is in Exercise Physiology. He also has degrees in Respiratory Therapy, Nursing and a Master Degree in Public Administration with an emphasis in Health Practice Management. He is currently licensed as a Respiratory Care Practitioner and Registered Nurse in California. Jim has worked in a variety of management and clinical areas that include: Cardiopulmonary, Neurodiagnostics, Cardiac Cath Lab, Electrophysiology Lab, Managed Care, Case Management and Disease Management Programs.
Sacramento CMSA is the local chapter of the Case Management Society of America (CMSA). CMSA is a nationally recognized innovative 501(c)(6) nonprofit organization which serves as the largest collaborative case management professional association in the United States. We embrace care management professionals from a variety of disciplines, practice settings, skill levels, and professional capacities, including:
Case Managers, Rehab Specialists, Students in CM programs, Care Managers, Physicians, Corporations Nurses, Health Coaches, Social Workers, Pharmacists, and other partners supporting the case management industry.
Definition of Case Management
Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality, cost-effective outcomes.
Philosophy of Case Management
The underlying premise of case management is based in the fact that when an individual reaches the optimum level of wellness and functional capability, everyone benefits: the individuals being served, their support systems, the health care delivery systems and the various reimbursement sources.
Case management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. The case manager helps identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for both the client and the reimbursement source. Case management services are best offered in a climate that allows direct communication between the case manager, the client, and appropriate service personnel, in order to optimize the outcome for all concerned.
Certification determines that the case manager possesses the education, skills and experience required to render appropriate services based on sound principles of practice.
