When it Comes to Readmission Reduction – Are You Working with the Right Home Care Provider?

Section 3025 of the Affordable Care Act added section 1886(q) to the Social SecuriAdvice_Nurse_webty Act created the Hospital Readmissions Reduction Program This law requires CMS to reduce payments to IPPS hospitals with excess readmissions.

The national average for people over 65 getting re-admitted back to the hospital within 30 days is 19.4‰. And, 3/4 of those folks will be back in the hospital within 3 days!

Using a professional Home Care agency like Harmony Home Care in Sacramento, El Dorado, or Placer Counties reduces the chances of readmissions to just 4.6%--a 75% reduction in risk!

The home care provider should have nursing supervision. Formulating a combined solution with a Home Health agency, developing a consistent care plan with the discharge planner at the hospital or nursing facility, and having the caregivers briefed on the particular diagnosis of the patient: these are the ingredients of the safest discharge possible. Our staff at Harmony Home Care has specialty programs for stroke patients, orthopedic patients, dementia and Alzheimer's care, falls prevention, and more.

Call us for a free consultation with our nursing supervisor. We can easily provide the date you need to have confidence that you are referring to the premiere home care agency in the Sacramento region.
