Elderly clients may not have current medical information readily available; Leaving out important medical information may lead to unnecessary tests and treatments. Having this information gathered and ready will help the EMS better assess the situation.

Some important information that should be readily available includes:

  • Medical condition(s)
  • Past surgeries
  • Cardiac diagnoses
  • Current medications with dosages and routes
  • Known drug allergies, including reaction
  • Names and phone numbers to family caregivers, physicians and specialists
  • Pharmacy contact information
  • Insurance Card (or copy)
